Rush Lost/Stolen Passport

STEP 1- Gather all the necessary documents

A. APPLICATION FORM DS #11 -(Click here) 
Please click the link above to fill out the application form.  Once completed please print.  You will notice the printed application will have a bar code on page #5 that appears on the top left hand corner next to the traveler’s name.  The State Department requires us to use this form as it allows for error detection and is easier to enter into their systems.

B. APPLICATION FORM DS #64 -(Click here) 
The DS #64 fills out when completing the DS #11 but only does so if you correctly indicate that your passport has been lost or stolen. If you do not mention this then the form does not populate. Please note that the State Department will know if you have had a previous passport in the past. Failure to report your passport by filling out and signing a DS #64 form will only succeed in causing delays.

C. Proof of citizenship.

  • Certified copy of birth certificate
  • A previous U.S. passport (expired over 5 years)
  • Naturalization certificate (original)
  • Minor passport that was valid for 5 years

D. 1 Passport sized photo

E. Proof of Departure

One of these:

  • Copy of airline tickets
  • Copy of itinerary or reservation
  • Letter on Company’s Letterhead (job or social institution)

F. Letter of Authorization

Authorization Letter:  Provided to you once your order is placed. Once your order has been placed we will then call you regarding your order and provide you with the proper authorization letter that will allow us to process your documentation in the time frame you have requested.

G. Proof of Identity

Valid driver’s license issued over 6 months ago. If driver’s license was recently issued please submit copies of the following 3-4 items:

  • Work picture ID with date of issue
  • Social or financial institution ID
  • High school diploma
  • Tax forms
  • Copy of public records with your name

H. Government Fees:

The government fees for a first time passport are $243.32. These fees have to be paid at the Passport Acceptance Facility. They are paid in the following manner: One check or money order for $208.32 and $35.00 in cash. If you chose to get the passport card in addition to the passport book please prepare a check or money order for $238.32. Your check must not be a starter check and must have your full name and address on it. Checks that do not have addresses are rejected by the Department of State.

STEP 2- Locate a passport acceptance facility closest to you and gather all the requirements in step 1

What happens at the Acceptance Facility? 

Advise the clerk that you are requesting the expedited process and using a third party service to do so. Please present our Authorization Letter. The clerk will review you materials to ensure that your documentation is correct and that you are the applicant. After your documents have been verified, the clerk will ask you to sign the application and then place everything in an envelope and seal it. This sealed envelope will be handed back to you for shipping and must remain sealed.

Applicants aged 16 & 17 must appear with one parent.
