To apply solely for a Name Change please note that this only applies to passports that have been issued for less than one year. If your passport has been issued for more than one year you must apply under the Passport Renewal requirements.
To apply solely for a temporary passport extension note that this only applies if the limited passport is still valid. If your passport has expired you must apply under the First Time Passport Requirements requirements.
You must also submit certified court documents showing the name change. The State Department may request previous certified documents in there have been previous changes to your name.
STEP 1 – Gather all necessary documents
A. APPLICATION FORM DS #5504 -(Click here)
Please click the link above to fill out the application form. Once completed please print. You will notice the printed application will have a bar code on page #5 that appears on the top left hand corner next to the traveler’s name. The State Department requires us to use this form as it allows for error detection and is easier to enter into their systems.
B. For Name Change Applicants: A Certified copy of Name Change documents
If you have changed your name multiple times please note the State Department may request additional certified copies of any previous changes to your name.
C. For Limited Passport Extension Applicants: Letter from the State Department indicating the next steps. If you are unable to provide this letter we can still apply but there may be some delays as not all limited passports are eligible for an extension.
D. 1 Passport sized photo

E. Proof of Departure
One of these:
- Copy of airline tickets
- Copy of itinerary or reservation
- Letter on Company’s Letterhead (job or social institution)
F. Letter of Authorization
Authorization Letter: Provided to you once your order is placed. Once your order has been placed we will then call you regarding your order and provide you with the proper authorization letter that will allow us to process your documentation in the time frame you have requested.
G. Passport that has been issued within the last year for a name change or has not expired for a limited passport extension.
H. Government Fees:
The process is free for regular processing of these passports. However, if you are needing expedite service the government fee is $82.05. This can be paid by check or money order. Your check must not be a starter check and must have your full name and address on it. Checks that do not have addresses are rejected by the Department of State.
